Patient studies in advanced kidney disease and heart failure often report:

• Treatment related adverse effects
• Poor quality of life
• High rates of clinical depression

Awareness of a patient’s experience and progress over time will allow:

• Individualized care plans
• Focused educational input from the multi-disciplinary team
• Evaluation of new treatments

A simple on-line, self reported Quality of Life and symptom indicator.

• Participants in the PD-HF Trial.

We aim to improve both patient and staff knowledge and understanding of quality of life and symptoms in kidney disease and heart failure.
